Is Testosterone Therapy Safe? What Current Research Shows
Is testosterone therapy safe? For appropriately selected men with confirmed hypogonadism, testosterone replacement therapy may be considered after a clinician reviews potential benefits, risks and monitoring needs. It is not risk-free, and it is not appropriate for every man with fatigue or a single low result.
What Counts as Low Testosterone?
Symptoms such as reduced sexual desire, erectile concerns, loss of body hair, lower muscle mass, low energy or depressed mood can have many causes. A diagnosis should not rest on symptoms alone.
The Endocrine Society guideline recommends diagnosing hypogonadism only when a man has compatible symptoms or signs and testosterone concentrations that are unequivocally and consistently low. The guideline also recommends confirming the result with a repeat morning fasting total testosterone measurement and evaluating the cause.
What the FDA Says About Cardiovascular Safety
The cardiovascular story has changed as stronger evidence became available. In February 2025, the FDA announced class-wide testosterone-labeling changes after reviewing the TRAVERSE cardiovascular trial and required blood-pressure studies.
The FDA removed boxed-warning language about increased major cardiovascular outcomes because the TRAVERSE trial did not show an increased risk of those outcomes in men treated for hypogonadism. However, the agency also required blood-pressure warnings because studies confirmed that testosterone products can increase blood pressure.
That does not mean testosterone is “heart safe” for everyone. Medical history, blood pressure, recent cardiovascular events and other risk factors still matter. Read the FDA testosterone-labeling update.
Important Risks and Monitoring Questions
Red blood cell levels
Testosterone can raise hematocrit, the percentage of blood made up of red blood cells. Baseline and follow-up monitoring may be needed, and an elevated result requires clinical attention.
Blood pressure
Because testosterone products can increase blood pressure, clinicians should consider baseline cardiovascular risk and monitor as appropriate.
Prostate evaluation
TRT has not been established as a cause of prostate cancer, but that does not remove the need for age- and risk-appropriate discussion, screening and follow-up.
Fertility
External testosterone can suppress sperm production. Men planning fertility in the near term should discuss this before starting treatment and may need a different approach or specialist input.
Sleep and breathing
Untreated severe obstructive sleep apnea is among the conditions the Endocrine Society lists as a reason not to start testosterone therapy.
Symptoms and side effects
Follow-up should assess whether symptoms improved, whether adverse effects occurred and whether the treatment remains appropriate.
Who May Not Be an Appropriate Candidate?
Contraindications and precautions require individual review. The Endocrine Society recommends against starting testosterone in men planning fertility soon and in patients with certain conditions, including breast or prostate cancer, elevated hematocrit, untreated severe obstructive sleep apnea, uncontrolled heart failure, a heart attack or stroke within the previous six months, or thrombophilia. Some prostate findings require urologic evaluation before treatment.

TRT Is Not the Same as Anabolic Steroid Misuse
Medical testosterone therapy is intended to treat a diagnosed deficiency using a prescribed plan. Anabolic steroid misuse often involves nonmedical use, unmonitored combinations or doses intended to produce effects beyond normal physiology.
The distinction does not make prescribed testosterone harmless. Product choice, dose, follow-up and patient-specific risk still matter.
A Safer Testosterone-Therapy Process
- Confirm compatible symptoms and consistently low testosterone. Repeat morning testing is commonly part of diagnosis.
- Look for the cause. The clinician may consider pituitary, testicular, medication, sleep, metabolic or other contributors.
- Review fertility goals and contraindications. These discussions belong before the first dose.
- Discuss realistic benefits, limitations and risks. No responsible clinician can guarantee a specific result.
- Build a follow-up plan. Monitoring depends on the patient, product, response and clinical guidelines.

Is testosterone therapy safe long term?
Long-term safety cannot be reduced to a yes-or-no answer. It depends on the diagnosis, personal risk factors, product, dose and ongoing monitoring. Patients should review benefits and uncertainties with their clinician.
Does TRT increase heart-attack or stroke risk?
The TRAVERSE trial did not show an increased risk of major cardiovascular events in its studied population, leading the FDA to remove related boxed-warning language in 2025. The FDA also added warnings that testosterone products can increase blood pressure, so individual cardiovascular assessment still matters.
Does testosterone therapy cause prostate cancer?
Evidence has not established that medically prescribed TRT causes prostate cancer. Still, clinicians should discuss prostate risk and appropriate monitoring based on age, history and individual risk factors.
Can TRT affect fertility?
Yes. External testosterone can suppress sperm production. Anyone who wants children soon should discuss fertility goals before treatment begins.
Do I need an in-person first visit?

What tests are required before TRT?
